We ask the retired and current professors (full-time / tenured / contractors), retired and current employees (tenured / contractors …) and students who wish to receive the vaccine through the Lebanese University to fill out the attached form.
The Lebanese University will send the application of anyone over the age of 65 and wishes to receive Pfizer vaccine, to the Ministry of Public Health.
Each member of the Lebanese University Teaching Staff Mutual Fund and the Employees Mutual Fund must fill out a separate form for those in their custody.
